• You are a regular patient of Vitalis Family Medical Practice
• It is deemed medical necessary and you cannot physically attend the practice
• You are located within 5 km from Vitalis Family Medical Practice

There is a Practice fee for a home visit. A list of these fees can be found under the Fees section.
